“There might be some bruising” isn't the whole safety conversation. You can ask what else could happen without being difficult or dramatic. The useful answers include warning signs, the product's source and who would help if something went wrong.

## Some symptoms need help now

After a botulinum toxin injection, blurry or double vision, eyelid droop, muscle weakness or difficulty swallowing or breathing need [emergency medical help immediately](https://www.cdc.gov/botulism/prevention/botulinum-toxin-injections.html). Symptoms can begin hours to weeks later. Time since the appointment doesn't rule out a problem.

During or shortly after a filler procedure, unusual pain, changes in vision or white, gray or blue skin near the treated area need [immediate medical attention](https://www.fda.gov/medical-devices/aesthetic-cosmetic-devices/dermal-fillers-soft-tissue-fillers). Don't wait for a website reply.

[Call 911 for breathing difficulty](https://medlineplus.gov/ency/article/000007.htm). Sudden vision changes, weakness or difficulty speaking can be stroke symptoms; [call 911 for suspected stroke](https://medlineplus.gov/stroke.html). You don't need to work out the diagnosis yourself before getting help.

## A temporary change can cause lasting harm

The two procedures have different serious risks. A toxin's effect can [spread beyond the treated area](https://www.accessdata.fda.gov/drugsatfda_docs/label/2024/103000s5316s5319s5323s5326s5331lbl.pdf), causing problems such as life-threatening breathing or swallowing difficulties. Approved medicine still carries this warning.

Filler can accidentally enter a blood vessel and damage the tissue it supplies. FDA describes [tissue death, blindness and stroke](https://www.fda.gov/consumers/consumer-updates/dermal-filler-dos-and-donts-wrinkles-lips-and-more), with harm that can be permanent. A temporary gel doesn't make that harm temporary.

Ask the clinician to explain the risks for the actual proposal and your medical history. “It's safe” isn't much to take home. You can ask for a clearer answer without having to memorize every possible complication.

## A license and a product are separate checks

In 2024, FDA linked harmful reactions, including hospitalizations, to counterfeit botulinum toxin. Its alert described incidents involving [licensed as well as unlicensed people](https://www.fda.gov/drugs/drug-alerts-and-statements/counterfeit-version-botox-found-multiple-states). A license alone doesn't establish where the medicine came from.

CDC's related investigation involved counterfeit or mishandled toxin and failures to meet local requirements. [That investigation closed in December 2024](https://www.cdc.gov/botulism/outbreaks/harmful-reactions-botox-injections/). Its findings help explain why product sourcing matters.

Ask for the exact medicine name and whether it came from an authorized source. CDC warns that [unsafe products can look like approved ones](https://www.cdc.gov/botulism/prevention/botulinum-toxin-injections.html). Appearance isn't a substitute for a clear answer about sourcing. Don't try to settle the question by obtaining a product to inject yourself.

## Hear the plan for a problem

For a filler-related blood-vessel complication, FDA tells providers to have [an updated treatment and referral plan](https://www.fda.gov/medical-devices/aesthetic-cosmetic-devices/dermal-fillers-soft-tissue-fillers). Ask who would act and where urgent care would happen. Then ask who answers after hours.

The [provider checks](/provider-checks#emergency-plan) help you put those questions together. If you need treatment, get care first. A [product-safety report](/contact#safety-report) doesn't arrange medical help.

You can pause because an answer is unclear. You can also pause because you simply don't want to go ahead. Neither needs a better excuse.
